Current students or alumni should initiate this form if your current institution is requesting to check your student records as a Prospective Athlete and/or you need a Good Disciplinary Standing report. You will complete the first section and the form will route to the appropriate division(s) on campus (Title IX, Diversity, and Inclusion and/or Student Development Office). You will receive a copy once the process is complete (ensure you enter your email correct). You must then send that copy to your requesting institution (it will not be sent on your behalf).

Resources for Pregnant and Parenting Students
The Office of Title IX, Diversity, and Inclusion is a resource on campus for pregnant and/or parenting students who have questions about their rights, reporting a violation of rights, or who need assistance with requesting accommodations.

Pregnant and parenting students are granted specific rights and protections through Title IX. 

Title IX of the Education Amendments of 1972 (“Title IX”), 20 U.S.C. §1681 et seq., is a Federal civil rights law that prohibits discrimination on the basis of sex—including pregnancy and parental status—in educational programs and activities.

This comprehensive assistance program is led by Generation Hope, an organization dedicated to ensuring all student parents have opportunities to succeed and experience economic mobility. 
The program aims to help students meet academic goals and graduate by offering guidance to parenting students through two years of mentoring and support.
