CARE Forms

Find forms and flyers available to CARE students.

Make sure to check EOPS Forms regularly to stay up to date with important EOPS forms and deadlines.

Below are various forms and flyers CARE students should access on a regular basis. Read the description for each document to help you select the correct document(s). If you have any questions or need a hard copies of any of the documents below, please feel free to contact us or stop by the CARE office.

CARE Certification Form New and continuing students must complete the CARE Agency Certification form once each academic year (beginning July 1). There are two steps to the process: 1. Complete the online 2024-2025 CARE Agency Certification Form. 2. Once the online eligibility form is completed, you will be contacted by a CARE staff member to go over your eligibility for the program and begin the intake process if all requirements are met. Important Dates and Resources All CARE students have access to supplemental resources throughout the semester. Review the Important Dates and Resources flyer to learn how and when you can receive CARE services.	View Document
CARE Workshops Every month this semester, CARE will be hosting "CARE Convos" - a virtual or in-person “hang out” with other parenting students. RSVP for as many of each - no penalty if you do not attend. Additionally, CARE staff promotes workshops hosted by campus and community partners. Check your ECC email and/or texts for more information.	Register here for "CARE Convos"
