Club Excursions

If your organization is going on a field trip or attending an off-campus conference, submit the documents below. Please note the deadlines. Late documents may be rejected.

• Student Travel Form (due four weeks before the excursion)
• Excursion Waiver Form (due one week before the excursion - NO EXCEPTIONS)
• Personal Vehicle Use Form (If you will be driving yourself to the excursion)
